Headgear is needed when arch wires, brackets or clear braces alone won’t achieve the desired result. Headgear is wiring that involves the inside and the outside of the mouth. Retraction headgear draws the upper jaw back, while protraction headgear moves the upper jaw forward to guide and stabilize the lower jaw.
The facebow model is a strap that wraps around the back of the neck, and the J hook model uses straps that fit over the neck and head. Both types use a wire that is hooked to the front of the braces. How long a patient must wear headgear depends on how crowded and/or asymmetrical the lower and upper jaws are, as well as how strictly the patient adheres to the orthodontist’s instructions.
Because headgear is detachable, it must be taken off while a patient eats, sleeps, engages in sports or any other kind of physical activity that might put its wellbeing in jeopardy.
Headgear isn’t feasible for adults receiving orthodontic treatment because it must take advantage of the fact that the jaw bones in a child or adolescent are still growing and pliable.
Wearing headgear requires more effort and attention from the patient than braces alone, but the extra work will be well worth it when the result is a healthy and beautiful smile that will last a lifetime.
